Initial Consultation

Your initial appointment at Superior Sleep and Wellness will consist of a consultation to discuss your medical history and sleep assessment patient questionnaire form. Please download, print and fill out this form, as well as our new patient form, at home and bring the forms into the office when you come for your appointment. We will also describe the sleep process and answer any and all questions you may have during this appointment.

Please assist us by providing your ID and medical insurance card(s).

Sleep Study
If you have previously received a home sleep test or polysomnography, please request to have those results forwarded to our office. If time is limited, please pick these documents up and bring them with you to your initial consultation.

Depending on when the sleep study was conducted, if the results are still valid we may be able to proceed in discussing your diagnosis and the treatment options available. Occasionally, treatment may be able to be performed on the same day as the consultation.

Prescription
If you have received a prescription for an oral appliance by a physician, please bring this prescription with you to the appointment as well.
